La Vaso-Vasostomie Pelvi-Scrotale pour obstruction inguinale du canal déférent: Revue de la littérature

Resume
Ces dernières années, le développement considérable des techniques d’aide médicale à la procréation (AMP) a modifié la prise en charge des couples infertiles, tant dans l’évaluation diagnostique que dans les possibilités thérapeutiques disponibles rendant moins fréquente l’exploration de la voie génitale en cas d’oligozoospermie. La cure de hernie inguinale, intervention fréquente, est une cause possible de l’obstruction du déférent. La prothèse synthétique utilisée pour renforcer la paroi déhiscente induit, comme tout corps étranger, une réaction inflammatoire aiguë plus ou moins intense, suivie par une réponse fibroblastique qui engaine le cordon spermatique.
Les cas d’azoospermie par obstacle déférentiel inguinal secondaire à une cure de hernie inguinale sont parmiles plus difficiles à reperméabiliser. Jusqu’à présent, on réalisait une vaso-vasostomie du déférent inguinal. Une nouvelle alternative de réparation de l’obstruction inguinale du déférent secondaire à une cure de hernie est proposée. Elle consiste à réaliser une Vaso-Vasostomie Pelvi-Scrotale (PSW) en court-circuitant le trajet inguinal du déférent par la coelioscopie. Le déférent pelvien est détaché de l’orifice inguinal profond, mobilisé en dedans, passé à travers l’orifice inguinal superficiel afin de faciliter l’anastomose avec le déférent scrotal.
Les auteurs concluent qu’il s’agit d’une technique simple, efficace, reproductible, réalisable de deux côtés dans la même séance opératoire et qu’elle offre un vrai espoir d’avoir des enfants de façon naturelle et spontanée chez ces hommes.
Abstract
Pelvi- scrotal vasovasostomy for inguinal obstruction of the vas deferens: Review of the literature
The recent development of assisted reproductive technologies has greatly modified evaluation and treatment of infertility and reconstructive surgery has been challenged over recent years by IVF techniques in the case of obstructive azoospermia.
Inguinal hernia repair is a frequent surgical procedure that may cause vas deferens obstruction. Polypropelene mesh induces an inflammatory reaction followed by a dense fibroblastic reaction surrounding the spermatic cord. Azoospermia secondary to mesh hernia repair is one of the most difficult cases for andrological reconstructive surgery. Up until now, open vasovasostomy has been the only available technique, but gives poor results. A new option called laparoscopic pelvi- scrotal vaso- vasostomy PSW consists of mobilization of the endopelvic vas deferens from the deep inguinal ring to the superficial ring, allowing tension- free anastomosis with the distal part of the vas.
The authors conclude that this technique is reliable, efficient and reproducible. It can be performed on both sides during the same procedure and it may open new possibilities for these patients to recover fertility and to father children naturally.
