Naissance après ICSI réalisée avec sperme éjaculé, congelé, chez un jeune patient de 21 ans porteur d’un syndrome de Klinefelter homogène
Birth after ICSI carried out with frozen ejaculated sperm from a 21-year-old patient with non-mosaic Klinefelter’s syndrome

Resume
Le syndrome de Klinefelter est l’une des principales causes génétiques d’infertilité masculine. Il est en effet diagnostiqué chez 11% des patients présentant une azoospermie et 4% des hommes infertiles [23].
Notre étude rapporte une naissance après ICSI utilisant du sperme éjaculé chez un jeune homme de 21 ans, présentant un syndrome de Klinefelter pur, homogène, découvert lors d’un bilan d’infertilité pour oligozoospermie sévère.
Sur une période de 18 mois, trois ICSI ont été réalisées pour ce couple. Au cours des deux premières tentatives avec du sperme éjaculé frais, 10 embryons au total ont été transférés sans obtenir de grossesse. Lors de la 3e ICSI, nous avons dû utiliser des spermatozoïdes éjaculés congelés, car le patient présentait une azoospermie avérée le jour de l’ICSI. Une grossesse a été obtenue après transfert de 3 embryons de grade A, avec naissance à ternie dépassé d’une fille en bonne santé.
Nous soulignons l’intérêt d’une congélation préventive répétée lorsque des spermatozoïdes éjaculés sont disponibles dans tous les cas d’oligozoospermie sévère ou de cryptozoospermie.
De plus, nous avons évalué la qualité des spermatozoïdes congelés et utilisables en ICSI par une analyse de leur ADN et une analyse de leur ploïdie par FISH.
Abstract
Klinefelter’s syndrome is one of the main genetic causes of male infertility, as it is diagnosed in 11% of patients with azoospermia and 4% of infertile men.
This study reports a birth after ICSI performed with ejaculated sperm from a 21-year-old man with homogeneous nonmosaic Klinefelter’s syndrome discovered during assessment of infertility for severe oligozoospermia.
Three ICSI were performed for this couple over an 18-month period. Pregnancy was not achieved after the first and second ICSI with fresh ejaculated sperm. At the third ICSI, the patient presented proven azoospermia on the day of the attempt, and frozen-thawed ejaculated spermatozoa were therefore used. A pregnancy was obtained after the transfer of 3 grade A embryos with the birth of a healthy girl.
The authors highlight the value of repeated preventive sperm cryopreservation when ejaculated spermatozoa are available in all cases of severe oligozoospermia or cryptozoospermia.
They also evaluated the quality (DNA fragmentation, ploidy) of the frozen/thawed spermatozoa.
