Les accidents de la circoncision: aspects anatomo-cliniques et thérapeutiques au CHU de Conakry, Guinée. A propos de 44 cas

Circumcision complications: clinicopathological and therapeutic aspects based on 44 cases at Conakry Hospital, Guinea

Resume

Objectif

Préciser les différents aspects anatomo-cliniques et thérapeutiques des accidents de la circoncision reçus en milieu hospitalier guinéen.

Patients et Méthode

II s’agit une étude rétrospective de type descriptif portant sur 44 accidents de circoncision colligés sur une période de 102 mois (8 ans 6 mois) dans les services d’Urologie-Andrologie et de Chirurgie pédiatrique du CHU de Conakry.

Résultats

L’âge moyen des patients í la réception était de 14,9 ± 8,3 ans avec des extrêmes de 10 jours et 32 ans. Le délai moyen de consultation en milieu spécialisé était de 16,8 ± 2,3 mois avec des extrêmes de 2 heures et 37 mois et seuls 29,6 % (n = 13) des patients ont consulté en milieu spécialisé durant la semaine de survenue de l’accident de circoncision.

La circoncision avait été effectuée par un agent paramédical dans 64 % des cas et par un tradipraticien dans 36%. Les lésions étaient dominées par les fistules urétro-cutanées (28 cas), les hémorragies péniennes (10 cas) et les amputations du gland (2 cas). Certains accidents étaient rares tels que la sténose du méat urétral et la rétention aiguë d’urine vésicale.

Conclusion

Les accidents de la circoncision sont graves et peuvent engager le pronostic vital. Leur prévention passe par une meilleure accessibilité des enfants aux structures sanitaires qualifiées, et aussi par la formation du personnel réalisant cet acte chirurgical.

Abstract

Objective

To determine the various clinicopathological and therapeutic aspects of circumcision complications observed in a Guinean hospital.

Patients and Method

This retrospective descriptive study was based on 44 circumcision complications observed over a period of 102 months (8 years 6 months) in the Urology-Andrology and Paediatric Surgery departments of Conakry University Hospital.

Results

The mean age of these patients on admission was 14.9 ± 8.3 years (range: 10 days to 32 years). The mean time to specialist referral was 16.8 ± 2.3 months (range: 2 hours to 37 months) and only 29.6% (n = 13) of patients consulted a specialist during the week following the circumcision complication.

Circumcision was performed by a paramedic in 64% of cases and by a traditional practitioner in 36% of cases. The main lesions were urethro-cutaneous fistulas (28 cases), penile haemorrhage (10 cases) and glans amputations (2 cases). Some complications were rare, such as stricture of the urethral meatus and acute urinary retention.

Conclusion

Circumcision complications are serious and can be life-threatening. Their prevention is based on a better accessibility of children to qualified healthcare structures, and better training of the persons performing this surgical procedure.
