Cryoconservation des tissus testiculaires: Quel avenir?
What is the future of testicular tissue cryoconservation?

Une augmentation globale de l’incidence des cancers est observée depuis vingt ans chez l’adulte comme chez l’enfant. Cependant, les progrès réalisés dans la prise en charge diagnostique et thérapeutique de ces cancers ont transformé une maladie fatale en maladie curable. L’amélioration de l’efficacité des thérapeutiques passe par une augmentation de leur toxicité. Un grand nombre de complications ou séquelles peuvent survenir chez ces survivants guéris d’une affection maligne de l’enfance, parmi lesquelles les altérations du tissu, gonadique sont de loin les plus fréquentes.
Chez la petite fille, la congélation du tissu, ovarien est devenue une pratique accessible et l’utilisation du tissu ovarien par greffe semble ouvrir des perspectives intéressantes. Chez le garçon pubère, l’autoconservation de spermatozoïdes éjaculés est envisageable, alors qu’en cas d’échec ou d’impossibilité de réalisation de ce recueil mais aussi chez le garçon non pubère, une autre stratégie doit être mise en œuvre: le prélèvement chirurgical du tissu testiculaire et sa congélation.
Les protocoles de congélation des gamètes mâles matures éjaculés sont bien établis. Cette méthodologie a été transposée secondairement sur les spermatozoïdes prélevés au niveau de l’épididyme mais aussi extraits de fragments de tissu testiculaire. Cependant, la congélation du tissu testiculaire adulte entier est moins répandue. Les protocoles de congélation du sperme éjaculé ont été élaborés pour les spermatozoïdes et ne sont pas adaptés aux cellules germinales immatures, les spermatogonies. Ces cellules riches en cytoplasme présentent une certaine vulnérabilité à la congélation. Ainsi, les protocoles de congélation restent à définir tant sur la composition du milieu de congélation, les temps d’incubation dans ce milieu, la courbe de descente en température que sur les modalités de décongélation.
L’utilisation ultérieure du tissu, testiculaire immature congelé définira la stratégie de congélation. Deux options peuvent être retenues: (i) la congélation du tissu testiculaire entier ou (ii) la congélation des spermatogonies en suspension. L’utilisation du tissu, testiculaire immature congelé pourra s’effectuer soit après maturationin vitro des cellules germinales, soit par transplantation des cellules germinales ou du tissu entier par greffe autologue voire suggéré par certains auteurs par xénogreffe. Cependant, cette dernière approche soulève des questions à la fois éthiques et biologiques.
Abstract
Over the last twenty years, an increased incidence of cancer has been observed in the population of adolescents and young adults. With the progress in cancer diagnosis and therapy, childhood cancer has become a curable disease. The efficacy of treatment is associated with a high degree of toxicity and gonadal function is particularly sensitive to this toxicity. Prevention of sterility in childhood cancer survivors will become a major challenge in reproductive medicine.
Cryopreservation of ovarian tissue is performed in girls and women before cancer treatment. Cryopreservation of ejaculated spermatozoa is possible in sexually mature boys. However, for prepubertal boys or after failure of ejaculated sperm collection, mature or immature testicular tissue banking should be proposed. However, an optimal cryopreservation protocol is a prerequisite for clinical applications. Furthermore, the future applications of immature testicular tissue banking should be developed, not solely autologous testicular tissue grafting, but also in vitro maturation of germ cells. Cryopreservation protocols, transplantation and in vitro maturation techniques should be improved in animal models and in humans.
