Résumé

Dans le domaine de la dysfonction érectile les enquêtes épidémiologiques sont récentes et dantent pour la plupart de la dernière décennie. Ces enquêtes comportent le plus souvent une question globale et un questionnaire détaillé de type IIEF (International index of erectile function). Réalisées dans de nombreux pays, elles sont toutefois difficiles à comparer entre elles car les méthodologies (parfois discutables) et l’interprétation des résultats diffèrent.

En France la prévalence de la dysfonction érectile modérée ou sévère est comprise entre 11 et 19%, en Europe (Allemagne, Espagne, Italie, Pays-bas et Suède) entre 3 à 11% et aux USA, selon l’enquête MMAS (Massachusetts male aging study), elle est évaluée à 35%.

Toutes les enquêtes de prévalence font par ailleurs apparaître une corrélation entre dysfonction érectile et âge: entre 40 et 70 ans le risque relatif de dysfonction érectile est multiplié par 2 à 4 selon les enquêtes et l’âge seuil semble se situer entre 50 et 60 ans. En France la prévalence de la dysfonction érectile modérée ou sévère après 60 ans est comprise entre 13 et 28%.

L’âge apparaît ainsi comme un facteur de risque prépondérant de dysfonction érectile.

En ce qui concerne l’incidence de la dysfonction érectile les données sont très pauvres et se limitent qusiment à l’enquête MMAS: grâce à deux évaluations effectuées à huit ans d’intervalle l’incidence annuelle de la dysfonction érectile a été évaluée à 25,9 cas pour 1000 hommes avec une augmentation avec chaque décade.

Abstract

Many studies have provided epidemiological data on erectile dysfunction over the last ten years.

Surveys performed in several countries in the world include a single question and a detailed questionnaire, usually the IIEF (International index of erectile function). However, the results are difficult to compare because of methodological differences.

Prevalence estimates of moderate or severe erectile dysfunction range between 11 and 19% in France, 3 and 11% in Europe (Germany, Italy, Netherlands, Spain and Sweden) and in USA, according to the MMAS (Massachusetts male aging study), this prevalence is 35%.

Prevalence estimates increase with age and the threshold is situated between 50 and 60 years.

Incidence data are lacking, but data have recently become available from the MMAS. According to this study, the annual incidence rate for erectile dysfunction is 25.9 cases per 1,000 men. This rate increases with each decade of age.
