Les possibilités de procréation pour les couples où l'homme est tétra ou paraplégique se sont améliorées cette dernière décennie à la fois grâce aux progrès effectués dans le traitement des troubles de l'éjaculation et grâce au développement de nouvelles techniques d'Assistance Médicale à la Procréation (AMP) dont l'ICSI. En effet, la qualité duu sperme est souvent altéré. Les indications d'AMP chez les hommes blessés médullaires sont comparables à celles des autres patients présentant une infertilité masculine. Pour guider l'équipe d'AMP dans le choix de la technique, un test de migration-survie associé à une étude morphologique des spermatozoïdes sont nécessaires selon les recommandations de l'OMS (2000) [19]. L'objectif est de privilégier les techniques d'AMP dites plus “légères” telle que l'insémination artificielle avec le sperme du conjoint (IAC) en première intention.

Les résultats en terme de grossesse portent souvent sur de petites séries. les données mettent en évidence des taux de grossesses cliniques par cycle comparables à ceux des patients non neurologiques.

L'autoconservation de sperme est à programmer assez rapidement dans la prise en charge du patient afin d' éviter de répéter des techniques de prélèvements invasives et douloureuses et par sécurité avant de programmer l'AMP.

Une étude multicentrique serait à envisager afin de réunir toutes les données des centres français.

Abstract

Spinal cord-injured men with ejaculation disorders can have children by means of Assisted Reproduction Techniques (ART). The possibilities of procreation of couples in which the man is praplegic or tetraplegic, have been improved over the this last decade by progress in the treatment of ejaculation disorders and the development of new ART techniques such as ICSI. Sperm quality is usually impaired in these patients. The indications for ART in spinal cord-injured patients are similar to those of other forms of male infertility. According to WHO recommendations, sperm preparation must be performed together with morphological study of spermatozoa to select the most appropriate ART technique, giving preference to so-called “simple” ART techniques, such as Intra-Uterine Insemination (IUI).

Small studies have been conducted and showed that the clinical pregnancy rate per cycle was comparable to that observed in non-neurological patients.

Autopreservation of sperm must be rapidly programmed to avoid repreated electrostimulation techniques and as a security measure before performing ART.

A multicenter study should be conducted to pool data from all French centres.
