Diagnosis and prognosis of partial obstructions of the seminal pathways

La cause d'une oligospermie a toujours été considérée comme résultant d'une spermatogenèse déficiente.

Une littérature abondante traite des différentes situations pathologiques perturbant la spermatogenèse à ses divers stades.

En dehors des causes testiculaires bien établies, un groupe important d'oligozoospermies est toujours resté méconnu: les lésions épididymaires sub-cliniques ne causant pas une oblitération totale des tubules mais un étranglement diminuant le transit et menant à un nombre réduit de spermatozoïdes dans l'éjaculat.

Cette situation d'oligozoospermie épididymaire ne peut être diagnostiquée que par une étude comparative des biopsies et des numérations dans les éjaculats.

L'analyse d'un grand nombre de cas (640) montre qu'un quart des oligozoospermies trouvent leur cause dans les perturbations du transit épididymaire.

Abstract

Poor spermatogenesis is generally considered as the cause of oligozoospermia and extensive literature has been devoted to this topic with regard to causes and eventual treatments. The role of the epididymis in causing oligozoospermia has been too frequently overlooked although it concerns the larger number of infertile males. The diagnosis is established by a quantitative comparison between the reading of the testicular biopsy and the sperm-count in the ejaculates. A complete study of over 640 severe oligozoospermic males established that 23% of them suffer from epididymal oligozoospermia.
