Résumé
1127 patients infertiles présentant une oligospermie < 5 millions/ml ont été pris en charge entre mars 1987 et janvier 1998. Parmi eux, 876 (âge moyen 35,2 ans) ont été suivis pendant une durée minimale de 1 an (suivi moyen 3,2 ans). Durant ce suivi, tous ont effectué au minimum 2 spermogrammes à des intervalles de 6 mois à 3 ans.
Nous avons étudié l'évolution de la qualité du sperme et tenté de dégager des critères pronostiques prédictifs de l'évolution de ces spermes au cours du temps.
On retrouve une tendance à l'aggravation des différents paramètres spermatiques au cours du suivi, et ce dès le 6ème mois. A 4 ans, plus de 2/3 des spermes se sont altérés, aucun ne s'est normalisé. Parmi les critères prédictifs d'altération, le % de formes mobiles apparaît être comme le plus important. Par ailleurs, la rapidité d'altération semble être inversement proportionnelle au % de formes mobiles.
En conclusion, même si le % de formes mobiles semble corrélé à la numération initiale, celui-ci semble être un facteur important pour apprécier une altération potentielle de la qualité du sperme avec le temps. Un % de formes mobiles inférieur à 20% est en faveur d'une altération significative dans les 2 ans de suivi.
Abstract
Between March 1987 and January 1998, 1127 patients with an oligospermia < millions/ml were followed up for infertility problems.
876 of these patients were followed up at least one year (mean: 3.2 years).
During this period, each patient performed at least two spermograms with an interval varying from 6 months to 3 years.
We studied the evolution of the quality of sperm during follow up and tried to show the pronostic factors of a potential decline
We found an impairment of all the sperm quality parameters from the outset of the sixth month. At the fourth year of follow up more than 2/3 of the patients had a significant impairment of sperm quality and none of them showed any improvement.
The % of sperm motility appears to be the most important prognostic factor in predicting an impairment. Sperm motility <20% predicts a significant impairment in sperm quality in the following two years.
