Techniques d’AMP et leurs indications dans l’hypofertilité masculine

Résumé
Malgré de multiples essais, le traitement médical de l’infertilité masculine reste décevant. A l’exception des traitements antibiotiques ou anti-inflammatoires, le traitement par les gonadotrophines dans le cadre des insuffisances gonadotropes demeure la seule indication médicale de l’infertilité masculine. L’assistance médicale à la procréation (AMP) constitue donc le plus souvent le seul recours au traitement de l’infertilité masculine. Les caractéristiques du sperme définies comme convenables pour obtenir la fécondation ont été déterminées statistiquement: pour la fécondation naturelle les normes définies par l’OMS en 1992 sont: numération égale ou supérieure à 20 millions par ml, mobilité progressive supérieure ou égale à 50% à la lère heure, morphologie présentant au moins 30% de formes normales. Les techniques d’AMP ne représentent passtricto sensu un traitement curatif de l’infertilité masculine mais une démarche médicale permettant aux couples infertiles de réaliser leur projet parental. Les techniques de préparation du spermein vitro en vue d’une AMP ont pour but de sélectionner des spermatozoïdes mobiles à morphologie normale, d’éliminer le plasma séminal ainsi que les débris cellulaires et les spermatozoïdes morts mais aussi d’induire la capacitation des spermatozoïdes. Le choix entre les différentes techniques d’AMP disponibles repose essentiellement sur le nombre de spermatozoïdes mobiles recueillis après préparationin vitro.
Abstract
In spite of several trials, medical treatment of male sterility is still deceiving. Only few indications are likely to gain from antibiotherapy, anti-inflammatory molecules or hormonotherapy. Most of the time, ART techniques remain the last issue in male sterility. In 1992, the WHO defined the threshold sperm criteria’s in order to obtain natural fertilization: sperm concentration ≥ 20 millions per ml., progressive motility ≥ 50% after liquefaction, and normal forms ≥ 30%. In vitro techniques used for sperm preparation before ART, are necessary for multiple reasons: selection of motile spermatozoa’s with normal morphology, suppression of dead spermatozoa’s and cellular fragments, elimination of seminal plasma, and induction of sperm capacitating process. Several ART techniques are now available and the result of in vitro sperm selection will help to choose the best technique in each indication.
