Résumé

La situation de la fécondation assistée avec sperme épididymaire est revue dans son ensemble. Les indications ainsi que les techniques de prélevement sont détaillées. Une revue la plus complète possible des résultats obtenus ce jour par toutes les équipes pratiquant ces approches thérapeutiques est jointe. Les auteurs y ajoutent leurs propres résultats. La discussion soulève les difficultés de compréhension de la mobilité des spermatozoïdes prélevés dans les têtes épididymaires, en contradiction avec les notions classiques de physiologie de l’épididyme. Les résultats globaux mondiaux font état d’un taux de succès de 10% de grossesses par tentative.

Summary

The total problem of sperm aspiration and assisted conception is surveyed. Both indications and techniques are reviewed. An up to date review of the results obtained in all the teams dealing with this problems is joined to this survey. The discussion deals with the surprisingly good sperm motility at epididymal levels were it should not be expected in normal epididymal physiology. The overall worldwide results offer a 10% pregnancy rate by this approach.
