Prévention des risques d’infertilité liés aux traitements antitumoraux dans le cancer du testicule
Semen cryopreservation in patients with testicular cancer before antitumoral therapy: Results of a 25 years experience at CECOS Paris Cochin

Résumé
Au cours de ces dernières années, les progrès de la thérapeutique anticancéreuse ont permis d’obtenir la rémission d’un nombre croissant de cancers. Cependant, lorsque le cancer atteint des hommes jeunes, les traitements utilisés entraînent dans la majorité des cas une stérilité temporaire ou définitive sans que l’on dispose à ce jour de moyens pour la prédire. Ce problème se pose de façon aiguë dans le cas du cancer du testicule devenu le cancer le plus fréquent chez l’homme jeune en âge de procréer car la production et la qualité du sperme est déjà faible avant tout traitement. Une mesure préventive avant tout traitement consiste à effectuer une cryoconservation du sperme. Le présent article essentiellement basé sur des données accumulées durant les 25 dernières années au CECOS Paris Cochin fait le point sur les modalités et l’intérêt clinique de cette procédure. Les résultats récents de l’utilisation des spermes conservés en AMP sont favorables: grâce aux nouvelles possibilités offertes par la fécondationin vitro, et la fécondation assistée par microinjection intraovocytaire de spermatozoïde (ICSI), un nombre croissant de grossesses a pu être obtenu y compris avec des spermes de très faible qualité. A la lumière de ces résultats, l’intérêt clinique de la cryoconservation du sperme d’hommes atteints d’un cancer du testicule est démontré: celle-ci demeure le moyen le plus sûr de permettre une paternité future.
Abstract
In recent years, chemo- and radiotherapy for cancer patients have become increasingly successful, and sustained remissions have been achieved. However in young men, most of the current therapies still presently induce temporary or permanent infertility without any means of prediction. This is also true in the case of testicular cancer which is now the most frequent cancer in young adults males. Moreover, this problem is particularly crucial in patients with a testicular cancer since they have a poor semen quality before any antitumoural therapy. The present paper largely based on data accumulated over 25 years at CECOS Paris Cochin reviews the modalities and clinical usefulness of semen cryopreservation in young men with testicular cancer. Recent use of cryopreserved semen samples through ART has provided a growing number of pregnancies. These new data strongly demonstrate that semen cryopreservation should be offered to all men diagnosed with a testicular cancer since it provides the only reasonable chance of establishing a pregnancy after therapy.
