Pour une approche psychosomatique de l’Andrologie

Resume

Par le biais de l’approche psychosomatique nous pouvons rencontrer en andrologie des patients qui se plaignent de n’importe quel trouble andrologique sans spécificité, bien que cette approche soit plus particulièrement utile quand les troubles ont tendance à devenir chroniques, ou quand ils sont associés à un vécu anxieux et/ou dépressif, à moins que celui-ci ne soit induit par eux.

Face aux symptômes fonctionnels cette approche a une spécificité particulière.

Les temps forts de la consultation d’andrologie psychosomatique sont décrits en insistant sur la particularité de l’interrogatoire „conversation”, avec le repèrage de l’histoire du malade, les faits marquants de sa vie, ses éléments psychologiques perturbants, l’appréciation de son comportement. La richesse de l’examen clinique et de son environment de „paroles” et un certain nombre de moyens thérapeutiques sont brièvement décrits: le médicament, les méthodes corporelles, et les méthodes psychothérapiques (la psychothérapie de soutien, la psychothérapie comportementale, les psychothérapies de couple, les psychothérapies psychanalytiques et la psychanalyse…)

Quand un ou des symptômes deviennent chroniques une vie émotionnelle subjective se greffe autour du substratum anatomique, favorisant l’installation du vécu psycho-émotionnel. C’est ce dont nous cherchons à tenir compte par l’approche psychosomatique.

Abstract

Through the opening offered by the psychosomatic approach, we encounter patients with all kinds of andrologic complaints, even quite unspecific ones. But we do know this mode of contact is particulary useful and fruitful when the troubles lean towards chronicity, or are linked to anxiety and/or depression, be that condition cause or consequence.

As far as functional symptoms are involved, there is a specific value to this approach.

The key-time of that consultant, i.e; psychosomatic andrology, will be described, with the particularity of a „conversation” instead of the mandatory anamnesis, meanwhile in his life, the possibly perturbing psychological events, as well as an attempt to evaluate the patient’s behaviour.

The richness of the clinical examination within this doctor’s induced environment of „words” coupled to some therapeutic means will be summarized: drugs, somatic and psychological methods (meaning all kind of psychotherapic techniques: support, behaviourist, couple, psychoanalytic or psychoanalysis…)

When symptom(s) tend towards chronicity, an emotional subjective life grafts itself on the anatomical substratum, thus creating the opening for psychoemotional affects.

This is the core we try to attain and assess through the psychosomatic approach.
