Success of auto-intracavernosal injections of prostaglandin E1 in cases of veno-occlusive dysfunction resistant to Papaverine

Abstract

Erectile failure due to veno-occlusive dysfunction (venous incompetence) is generally thought to be resistant to auto-intra-cavernosal injection therapy. Because PGE1 recently emerged as a drug more potent than Papaverine, we tested it in 26 impotent patients presenting this condition. All of them had abnormal Nocturnal Penile Tumescence and Rigidity recording, a non rigid response to intracavernosal injection (ICI) of 80 mg papaverine, and abnormal pharmacocavernosometry (maintenance flow rate exceeding 30 ml/mn after 80 mg of papaverine). When tested with 20 μg of PGE1 and observed for 30 mn in our office, 16 out of 26 developped a rigidity seeming consistent with vaginal intromission, including full rigidity in 2. In several additional patients, partial or full rigidity was achieved after leaving the center. The patients who did not develop rigidity were retested with high dosages. Finally 19/26 developped a rigidity seeming consistent with vaginal intromission within 2 hours of an ICI of 20 to 60 μg of PGE1. Seven of those patients elected to try auto-ICI of 20 to 60 μg of PGE1. The treatment failed in 2. The 5 other ones were able to have intercourse after each ICI, but not without ICI. 2 achieved full rigidity, and 3 only partial rigidity. Papaverine failed in everyone. Until now they have performed about 250 ICI of PGE1, and the maximum follow-up is 18 months. No complications has been observed, including no priapism and no fibrotic nodule. PGE1 widens the possibilities of the auto-ICI therapy. It seems the first medical treatment effective in case of erectile failure due to severe veno-occlusive dysfunction, or at least the most powerful.

Résumé

on considère généralement que l’impuissance par dysfonction veino-occlusive ne répond pas au traitement par auto-injections intra-caverneuses. Dans la mesure où la Prostaglandine E1 (PGE1) est récemment apparue plus puissante que la Papavérine, nous l’avons testée chez 26 impuissants présentant ce problème. Tous avaient une pléthysmographie nocturne avec rigidométrie anormale, une réponse non rigide à l’injection intra-caverneuse (IC) de 80 mg de Papavérine, et une pharmaco-cavernométric anormale (débit de maintien de l’érection > 30 ml/mn après 80 mg de papavérine). Lorsqu’ils ont été testés avec 20 μg de PGE1 et observés pendant 30 minutes dans notre centre, 16 des 26 sujets installèrent une rigidité paraissant compatible avec la pénétration. Dans plusieurs cas supplémentaires, une rigidité partielle ou complète apparut après qu’ils aient quitté le centre. Les sujets qui ne présentèrent pas de rigidité furent testés de nouveau avec des doses plus élevées. Finalement, 19 sujets présentèrent une rigidité paraissant compatible avec la pénétration dans les deux heures suivant une IC de 20 à 60 μg de PGE1. Sept de ces sujets essayèrent des auto-IC de 20 à 60 μg de PGE1. Le traitement échoua dans deux cas mais les cinq autres furent capables d’avoir des rapports après chaque injection, dont deux avec rigidité complète. Jusqu’à maintenant ils ont fait environ 250 injections de PGE1 et le recul maximum est de 18 mois. Il n’a pas été observé de complications, ceci incluant l’absence de priapisme et nodule fibreux. La Prostaglandine E1 élargit manifestement les possibilités du traitement par auto-injections. C’est le premier traitement médical efficace dans des cas de dysfonctions veino-occlusives sévères, et en tous cas le plus puissant.
