Abstract

Since 1982, many substances have been injected intracavernosally to produce erections in impotent men. Because the first-generation drugs (phenoxybenzamine, papaverine) were associated with severe side-effects, including priapism and penile fibrosis, it was essential that a safe and effective substance be found for this application. In a series of 980 patient with erectile dysfunction who received prostaglandin E1 intracavernosal injections, 70 % developped an erection lasting more than 30 minutes with minimal side-effects. All the 510 men who entered a self-administration protocol were capable of engaging in intercourse and experienced no major sideeffects. Prostaglandin E1 is effective, safe and preferable to all other drugs currently used for intracavernosal injection.

Depuis 1982, de nombreuses substances ont été utilisées en injections intracaverneuses pour induire l’érection. Du fait que les composés de première génération (phenoxybenzamine, papavérine) indulsaient des effets indésirables sevères, incluant priapisme et fibrose du pénis, il était capital de trouver un composé qui soit à la fois sans danger et efficace pour les injections intracaverneuses. Dans notre sene de 980 patients avec impuissance ayant reçu une injection intracaverneuse de prostaglandine E1, 70% ont présenté une érection durant plus de 30 mn. Les effets indésirables ont été minimes. Cinq cent dix patients sont entrés dans un programme d’auto-injections. Tous ces hommes furent capables d’avoir des rapports, et ne présentèrent aucun effet indésirable important. La prostaglandine E1 est efficace, sans risque important, et préférable à toutes les autres drogues actuellement utilisées pour les injections intracaverneuses.
