Palliative treatment and impotence: Intracavernosal inections or vacuum?

Abstract

Two of the most common treatments for impotence are compared. While vacuum therapy has been very successful in the United States and other anglo-saxon countries, fear of priapism, fear of injections, and the existence of sequelae such as nodules or haematomas have been responsible for a great many patients abandoning intracavernosal injections. Howerver, the avaibility of “pens” and other self-injections devices have considerably reduced the prevalence of such complications and made the technique more acceptable. Use of new drugs and pharmacological combinations has also reduced the risk of priapism. The mode of action and different types of vacuum devices are reviewed, along with their advantages and disadvantages, and the results and complications of their use summarized from various published clinical series.

VACUUM ET INJECTION INTRACA VERNEUSE. L’auteur essaye de comparer entre 2 méthodes artificielles actuellement très utilisées. Le vacuum a un très grand succès aux Etats-Unis et dans les pays anglosaxons. La peur du priapisme, la peur de piquer la verge, l’existence de nodules, ou d’hématomes séquellaires sont responsables d’un grand nombre d’abandon des auto-injections. L’apparition du stylo et de l’autoinjecteur ont considérablement diminué ces complications et les bandons de la technique. L’utilisation le nouvelles substances et d’associations pharmacologiques a également diminué le risque de priapisme. L’auteur passe en revue le mécanisme de l’érecteur à dépression, les différents modèles, avec leurs avantages et leurs inconvénients, et fait la revue des résultats et des complications des différentes séries publiées.
