Therapy for erectile dysfunction: The discrepancy between availability and request

Abstract

Treatments usually offered to impotent men include psychotherapy, sex therapy, self-administered intracavernosal injections vacum therapy and penile prostheses. However, these treatments do not meet the expectations of many patients and cause them significant problems. This discrepancy between available treatments and patients, expectations explains the high level (over 50%) of therapy rejection, which probably has significant psychological and marital consequences. Consequently, it is preferable to offer intermediate and progressive forms of therapy which may comprise a series of interviews combining a seemingly traditional medical approach (prescription of “symptomatic” medications such as androgens, chorionic gonadotrophin or yohimbin and, in the case of failed office-based intracavernosal injections, vasoactive agents) with a verbal approach. The medical element increases the willingess to accept a psychogenic diagnosis in resistant patients. By allowing a patient more time to come to terms with the situation, by aiding realization of the underlying causative factors, and by helping patients become accustomed to the concept of self-administered injections. this bimodal approach increases both the acceptance level and probably also the success of treatment

Les traitements habituellement offerts aux impuissants sont les psycho et sexothérapies, les auto-injections intracaverneuses, les systèmes à vide et l’implantion de prothèses péniennes. Ces traitements ne répondent pas à l’attente de beaucoup des patients, et leur posent d’importants problèmes. Ce décalage entre l’offre et la demande explique le taux élevé plus de 50 %) de refus de traitement, lequel a probablement des conséquences psychologiques et conjugales importantes. Il est préférable d’offrir des solutions plus nuancées et progressives. Celles-ci peuvent prendre la forme d’une série de consultations combinant une approache médicale d’allure traditionnelle (prescription de médications “symptômatiques” type androgènes, gonadotrophines chorioniques ou yohimbine, et en cas d’échec d’injections intracaverneuses au cabinet de substances vaso-actives) à une approche verbale dont la précédente facilite l’acceptation chez les patients resistant au diagnostic de psychogénicité. En donnant plus de temps au patient pour s’adapter à la réalité en facilitant la prise de conscience des facteurs déterminants sous jacents, et eventuellement en accoutumant au principe des auto-injections, cette approche bimodale augmente le taux d’acceptation et probablement le succès du traitement.
