Psychological and marital factors in impotence: Always with us

Abstract

Although many cases of impotence are associated with “hidden” organic lesions undetectable by normal clinical assessment, and only detectable by paraclinical examination, it is obvious that interactions between psychological and factors often prevail in such cases. The paper describes the main psychological factors implicated in impotence, as well as recent data wich start to provide an explanation of the mechanisms by which psychological factors perturb sexuality. At the peripheral level, stress-induced sympathetic hyperactivity probably inhibits erection by causing contraction of the cavernosal smooth muscle. At a central level, animal experimentation and results from the pharmacological treatment of human impotence, suggest that psychological factors might inhibit sexual drive, and perhaps erection, through α 2—adrenergic receptors and the hypersecretion of endogenous opioids. These data open some highly stimulating pharmacological pespectives.

Résumé

Bien que beaucoup d’impuissances soient associées à des anomalies organiques “cachées” non discernables cliniquement, et ne pouvant être détectées que par des examens paracliniques, il est évident que le plus souvent, I’interaction de facteurs psychologiques et conjugaux continue de prévaloir dans de tels cas. L’article décrit les principaux facteurs psychologiques impliqués dans l’impuissance, ainsi que les données récentes fournissant un début d’explication sur les mécanismes par lesquels ces facteurs psychologiques peuvent perturber la sexualité: au niveau périphérique, une hyperactivité sympathique résultant du stress inhibe probablement l’érection en contractant le muscle lisse caverneux. Au niveau central, les données de l’expérimentation animale et les résultats des traitements pharmacologiques de l’impuissance suggèrent que les facteurs psychologiques pourraient inhiber l’intérêt sexuel et peut être l’érection par l’intermédiaire de l’action des récepteurs alpha 2 adrénergiques et par l’hypersécrétion de substances opioides endogènes. Ces données ouvrent des perspectives pharmacologiques très stimulantes.
