Varicocele

Abstract

Whether varicocoele should be treated while the patient is young remains controversial in spite of the high prevalence of this problem and the fact that it is believed by many to be a causative factor in male subfertility. Varicocoele is mostly asymptomatic and the diagnosis is made nowadays during routine clinical examinations. Döppler examination confirms Valsava-induced reflux and ultrasonography can detect intrascrotal abnormalities and allow accurate visualization of testicular size. Increase in retrograde venous flow, with decrease in testicular volume, are two proven mechanisms. Histological lesions worsen with time and disturbance of spermatogenesis become prominent with passage through puberty. Treatment of varicocoele is indicated in cases where the varicocoele is larger, if it is symptomatic, in youths with arrested testicular growth, and when a varicocoele enlarges with time. Two treatment options exist: a direct surgical approach or endovenous occlusion. Whichever method is employed, results must be confirmed by ultrasonographic and Doppler examinations.

La découverte d’une varicocèle à l’âge pédiatrique est souvent réalisée à l’occasion d’un examen systématique. La confirmation de la pathologie sera assurée essentiellement par l’échographie associée à l’étude par effet doppler du système veineux. La responsabilité du reflux veinospermatique et l’hypotrophie testiculaire secondaire sont des éléments physiopathologiques couramment admis. Les lésions testiculaires sur la lignée exocrine sont progressives. Une varicocèle volumineuse, symptomatique, persistante, ayant entraîné une diminution du volume testicularie doit être corrigée. Deux méthodes peuvent être utilisées: soit l’abord chirurgical par l’étude de l’effet doppler.
