Peyronie’s Disease: epidemiological aspects

L’épidémiologie de la maladie de La Peyronie est peu connue pour différentes raisons: la maladie est peu fréquente, elle ne met pas en jeu le pronostic vital enfin il existe une réticence d’ordre culturelle à consulter pour cette maladie. Cet article analyse d’après revue de la littérature les données d’épidémiologie descriptive et analytique existantes. L’incidence standardisée est de 25,7 pour 100000, la prévalence est de 388,6 pour 100000. L’incidence maximale se situe dans la tranche d’âge 50–59 ans. Les symptômes de découverte, localisation de la plaque, évolution spontanée sont également rapportés.

Les données d’épidémiologie analytique provenant de populations contrôlées ne retrouvent que deux facteurs de risque qui semblent significativement associés à la maladie de la Peyronie: l’arthrite (p<0,0001) et l’hypertension artérielle (p=0,013). Plus récemment, des auteurs ont rapporté des fréquences significativement plus élevées d’antigène HLA-DQ5 (p=0,005), HLA-DR3 (p<0.005) et DQw2 (p<0.0005) chez les patients porteurs de la maladie par rapport à la population générale.

Il n’existe pas de données actuellement disponibles dans le domaine de l’épidémiologie évaluative pour la maldie de la Peyronie.

Abstract

Epidemiological aspects of Peyronie’s disease are herein discussed. Medline research has been done from 1980 through 1996, only few articles have been found. The lack of data seems to be related to the low incidence of the disease, to the difficulty for the people to seek medical help for problems related to the area of sexual function, to the fact that the disease is not lethal.

Descriptive epidemiological data available noted an average age adjusted annual incidence rate of 25.7 and a prevalence rate of 388.6 per male population. Incidence by age group has been also analyzed and the highest incidence is reported for the 50 to 59 year age group. Symptoms of discovering, location of curvature, natural history and practice trends are also briefly reported.

Analytical epidemiological data available showed that rheumatoid arthritis (p<0.0001) and hypertension (p=0.013) were possible risk factors when comparing prevalence of these diseases found in Peyronie’s disease patients to expected number derived from the prevalence in the general population. Other risk factors were not proved. More recently autoimmune disorders have been found associated with Peyronie’s disease. HLA-DR3 (p<0.005), Dqw2 (p<0.005), HLADQ5 (p=0.005) frequency was significant increased in patients with Peyronie’s disease compared to age matched and general population controls.

No results are available about evaluative epidemiology.
