Indications d’une étude méiotique dans l’infertilité masculine

Resume

L’indication d’une exploration méiotique est rarement posée au cours de l’investigation d’une infertilité masculine. Or un certain nombre de mutations qui affectent le déroulement de la spermatogenèse se manifestent par des images caractéristiques lors de l’étude cytogénétique de la méiose alors que l’histologie testiculaire ne montre aucun aspect spécifique de ce type de pathologie. Dans le cas des infertilités à caryotype somatique anormal l’investigation méiotique a pour but d’étudier les mécanismes responsables de l’arrêt de la maturation des cellules germinales, et conduira à conduira à une meilleure connaissance de la spermatogeneèse normale. L’étude de la méiose, outre la technique cytogénétique, fait aussi appel aux techniques de microscopie électronique et de biologie moléculaire.

Nous pensons done qu’il faut accorder une plus large place à l’étude de la méiose dans le bilan d’une infertilité masculine, lorsque l’indication d’une étude histopatologique testiculaire a été posée.

Abstract

Meiotic investigation is rare in male infertility. Now, some mutations affecting spermatogenesis exhibit characteristic cytogenetic figures, whereas testicular histology does not show specific aspects of this pathology. In male infertility with abnormal somatic caryotype, the aim of meiotic survey is to find the mechanisms inducing spermatogenic failure, and thus to lead to a better understanding of normal spermatogenesis. In addition to cytogenetic techniques, meiosis is also investigated by electron microscopy and molecular biology.

Also, we think that a larger place must be grant to meiotic study in male infertility evaluation when the indication of testicular histopathology was settled.
